For example let's combine this palette and pattern below
Man_Brigade New_Nikes

First of all you need to have your browser's address bar visible. (also called location bar or URL bar ) How? Each browser is a little different so you'll need to Google it with the name of your browser.

On the pattern page, below the pattern, click on 'create pattern' - You'll get the following URL - notice the patternID at the end
www.colourlovers.com/patterns/add?patternDefinitionID=23941
real example

On the palette page, click on 'create pattern' - You'll get the following URL with all the colors are listed in HEX code
www.colourlovers.com/patterns/add? colors=06070B,136982,6D6E8D,662126,484B78&pID=1273152
All on one line - real example

To combine them, all you have to do is add up the 2 endings, link them with a '&' like in the example just below
www.colourlovers.com/patterns/add? colors=06070B,136982,6D6E8D,662126,484B78&pID=1273152 &patternDefinitionID=23941
All on one line - example

Once you have the palette code and pattern code in the address bar like in the above screenshot, press the keyboard [ENTER] key or the 'Go' button by the address bar to validate.

IN SHORT

Add up the patternID string &patternDefinitionID=23941 (that you went to fetch earlier) to the end of the create palette URL

Voilà! Simple! =)

Quick Palette Creation:
This technic also enables to create palettes directly typing the hexcodes - check out this URL
Could be useful if you have the hexcodes and want to quickly create a palette.

Thanks so much!I've been wondering how and wanting to do this but didn't even know what question to ask in regards to same. :) Ketisse directed me to this group, and I am certainly grateful she did!
